Normally, ejaculation is painless and a pleasurable sexual experience. When a man reaches orgasm and ejaculates, the lumbosacral spinal cord ejaculatory center controls the contraction of muscles in the epididymis, vas deferens, seminal vesicles, and prostate gland. This process discharges fluid from various organs into the posterior urethra, while simultaneously closing the bladder neck, perineal muscle, bulbocavernosus muscle, and ischiocavernosus muscle, leading to the expulsion of semen through the urethra.

However, if testicular pain occurs after ejaculation, it may indicate a male genitourinary tract infection. Common conditions include prostatitis, epididymitis, orchitis, and others.

The reasons for post-ejaculation pain can be:

1. Chronic Prostatitis: Patients may experience urethral irritation, frequent and urgent urination, burning pain, and the presence of mucus or purulent secretion at the urethral orifice in the morning. Discomfort can occur in the posterior urethra, perineum, anus, etc., along with ejaculatory pain, blood in the semen, premature ejaculation, impotence, fatigue, dizziness, insomnia, and depression.

2. Epididymitis: The affected side of the scrotum may feel swollen, painful, and heavy. Pain in the lower abdomen and groin can worsen while standing or walking. The epididymis on the affected side may be enlarged and tender. If inflammation is extensive, both the epididymis and testis may experience swelling, referred to as epididymitis.

3. Excessive Sexual Activity: Frequent sexual activity can cause aseptic inflammation in the vas deferens, seminal vesicles, and urethral organs, leading to congestion and edema. During ejaculation, the smooth muscle contraction may stimulate these congested organs, causing pain. Rough and aggressive sexual activity can also lead to ejaculation pain. Frequent sexual intercourse, especially during the newlywed period, may result in soreness in the perineum or scrotum.

4. Genital Tumors: Malignant tumors in the male genitourinary organs can cause ejaculation pain. Tumors in the testis, epididymis, prostate, and other locations may compress or block the ejaculatory duct, resulting in pain during ejaculation. 5. Genital Inflammation: Lower urinary tract infections are a common cause of ejaculatory pain in males. Some patients may also experience painful spermatorrhea.

6. Male Urethral Calculus: Urethral calculi are relatively common in men. Small stones embedded in the posterior urethra, especially in the open part of the ejaculatory tube, can be squeezed during ejaculation, leading to pain. If stones are present in the prostate or seminal vesicle, their contraction during ejaculation can also cause pain.

7. Psychological Factors: Some individuals may experience ejaculation pain due to anxiety about ejaculation quality, past sexual failures, work-related stress, etc., leading to spasmodic contractions of perineal muscles, particularly tension in psychological factors.

It is essential for patients to seek timely treatment for ejaculation pain caused by these factors, especially when it is related to reproductive system diseases like prostatitis, epididymitis, orchitis, etc., to prevent worsening conditions. Bacterial-induced diseases require treatment with antibiotics, and quinolones such as levofloxacin, clofibrate, or cephalosporins like cefixime and cefdinir are the preferred choices.
