How to add beneficiary in IFHRMS - (2021 Full Guide)

In order to add a beneficiary in IFHRMS, go to the 'Manage my plan' page and click on the edit button next to 'Family Member'.

You'll be directed to a screen where you can input all of the information for your beneficiary. Please note that only one beneficiary can be selected at a time. Once you have chosen a beneficiary, click on save and they will automatically be added.

Adding a new person will automatically remove the old person from your list of beneficiaries. It's important to note that removing a beneficiary will also remove this account from their Flex or Consolidated Omnibus Budget Reconciliation Act (COBRA) Medical Plan.

If you're removing your spouse or domestic partner, you would need to add yourself back in as the only family member in order to continue receiving coverage under the same plan.

To add another person, simply repeat these steps with another individual's information. Please note that only one person can be added at a time. These steps can be skipped if you want to add yourself back in as the sole beneficiary of your plan.

Select the "Add a Beneficiary" button

Fill in the first name, last name and address of your beneficiary. Enter your relationship to the person as "child, grandchild, other". You should also indicate that you are their primary caregiver. Press “Save” to submit this information.

(+Benefit from Medicaid) For those who have been deemed eligible for Medicaid benefits because of a disability or certain life circumstances, including being over 65 or living below poverty level, you may designate a beneficiary who will receive benefits if you die before they do.

How to add a co-beneficiary

To add a co-beneficiary, you must first select the "Manage my plan" option from your site followed by clicking on "Edit".

At this point you will be presented with the following screen:

Fill in the beneficiary's name, their relationship to you, and indicate that they are their primary caregiver. This step is important as it allows them to continue receiving coverage under your plan if one of you are no longer alive.

Select the save button when completed.

Fill-In (Address/email)

Enter the address or e-mail of the facility you wish to search

Select “search”. You should receive a list of hospice facilities in your area by county, zip code and facility name.

Now you can add beneficiaries. Please log in using your User name and password here: https://www.ifhrms.com/ifhrms-for-providers/login/ and then click on "Add a beneficiary". If you already see the beneficiary names in the list, then give them a name and save the changes.

Give your beneficiary at least one address and their relationship to you. You can add more addresses or change this information later. Your beneficiary may also write a letter to the hospice by simply clicking on their name and type "Letter" in the text box, and save changes.

You may also upload photos of your loved ones, by clicking on their names or simply click on "slideshow". You can add "slideshow" as many photos as you want. If you are new, please read some instructions first before uploading photos for your first time.

Beneficiary(ies) are caregivers or loved ones who will be notified on behalf of your wishes if you are ever hospitalized, at the end of life, or in a situation where you are unable to make decisions on your own.

Beneficiaries can also view your treatment history, hospice history and advanced directives on the hospice website. It is important to ensure beneficiary information is current and up-to-date.

You should visit: http://www.ifhrms.com/ifhrms-for-providers/login/ to log in. If you already see your beneficiary names in the list, then give them a name and save the changes.

Beneficiaries can write letters to the hospice by simply clicking on their name, type “Letter” in the text box and save the change.

You may also upload photos of your loved ones, by clicking on their names or simply click on “slideshow”. You can add “slideshow” as many photos as you want. Please read some instructions first before uploading photos for your first time.

You can always view and print a copy of the documents online, and the documents become part of your medical records and will be available to you.

If you visit your doctor or hospital, he or she can access these documents as well. All your information is kept secure and confidential through the use of a username and password access.

Family members can also view your treatment history, hospice history, care plan, advance directives on the hospice website. It is important to ensure IFHRMS beneficiary information is current and up-to-date.
