 The epididymis and the testes of men are vital and fragile organs. The epididymis and the testes are rich in blood vessels, varicocele, and interstitial tissues, and the innermost white membrane is distributed with a highly concentrated density of nerve endings. It is also connected to the nerve seed tree, which is extremely sensitive to pain perception. Infections of male epididymal tissues by pathogenic bacteria can cause swelling and pains in the epididymis.

For patients with epididymitis, choosing the right underwear can support the scrotum and alleviate the painful discomforts of the epididymis. Inappropriate underwear can increase the risk of epididymitis.

First, underwear without a capsular bag design can increase the risk of epididymitis.

The capsular bag underwear, also known as scrotum support underwear, is a complete separation of penis and scrotum, allowing for "gun and bullet separation." These capsular bags are designed to conform to the human anatomy, placing the penis in the U compartment and the scrotum in the capsular bag. 

Thus, men do not feel any pressure, preventing the scrotum from sagging and blood stagnation and the scrotum from rubbing against the thighs, which can increase ventilation and breathability. 

The capsular bag underwear can be used to assist in treating epididymitis, orchitis, varicocele, spermatorrhea, and other male diseases and alleviate the symptoms of patients.

Secondly, too heavy underwear may also induce epididymitis.

The design of most men's underwear on the market is double-layered in front, wrapping the penis and scrotum together, resulting in the penis and scrotum being in a hot and humid environment for a long time, which provides a backdrop for the breeding of germs and other microorganisms, easily triggering prostatitis and leading to epididymitis. However, it is often ignored by people.

Thirdly, triangle underwear is not suitable for epididymitis patients.

Compared with tight triangle underwear suitable for athletes, epididymitis patients prefer wearing loose and breathable boxer shorts. The underwear wraps around the scrotum and testicles, and the flexible underwear environment makes it easier for the scrotum to self-regulate. The scrotum is sensitive to external temperatures and has no subcutaneous fat or rich sweat glands. Wearing boxer shorts helps dissipate the heat and makes men more comfortable and healthy.

Wearing the wrong underwear may also suffer from epididymitis, so men should pay attention to the following aspects when choosing underwear:

1. Sizes of underwear:

For the most intimate clothing, the right size is most important for underwear. If the size is too small, it will cause discomfort in men, affecting the normal flow of blood in the perineum and easily causing sperm damage; when the size is too large, it will cause unnecessary friction and even make the private parts painful, so men must choose the right size. Men can choose the size according to the waist and thigh circumference.

2. Materials of underwear:  

Men should choose breathable cotton underwear and avoid underwear made of ice silk and chemical fiber because these materials have poor breathable and sweat-absorbing properties. Male scrotum easily sweats, so those materials with poor breathable and sweat-absorbing properties will cause discomfort in men, easily aggravating the epididymal pain discomforts.

3. Colors of underwear:

From the perspective of health, light-colored underwear should be chosen. Patients with epididymitis or prostatitis may have increased urethral secretions, and wearing light-colored underwear can help observe related conditions timely from the abnormal secretions on the underwear.

4. Cleaning the underwear timely:

The dirty underwear will carry a small number of fecal residues and many bacteria, such as salmonella and E. coli, which will accumulate on the surface of the underwear and enter the epididymis or testicles through the urethra, leading to inflammation of the epididymis and testicles.
