New research published in the journal Mental Health and Physical Activity highlights concerns about the scientific reviews conducted on exercise-based interventions for addiction. The study argues that methodological issues and knowledge gaps in the existing literature hinder the development of clinical recommendations in this area.

Substance abuse and dependence are significant public health concerns with high prevalence rates and associated risks such as suicide and mortality. These conditions are characterized by compulsions, chronic relapse, and often co-occur with other disorders like chronic pain or mental illnesses. Managing psychological symptoms and substance abuse outcomes in these populations is challenging and negatively impacts their quality of life, as well as economic, judicial, and health systems.

Exercise has been proposed as a potential intervention to support individuals with substance abuse or dependence. Regular physical activity may enhance self-discipline and help individuals cope with their symptoms. Javier Martínez-Calderón, study author and assistant professor at the University of Seville, emphasizes the importance of exercise-based interventions for mental health and well-being, particularly in the field of addictions. He believes that exercise can help individuals connect with themselves, improve their mental health, and reduce the abuse of substances.

Previous systematic reviews have shown promising results regarding the effectiveness of exercise-based interventions. However, there is still uncertainty about the most beneficial types of exercise for specific populations and the specific psychological symptoms or substance abuse outcomes that can be effectively addressed through exercise.

To shed light on this issue, the researchers conducted a comprehensive search of multiple databases and identified 18 systematic reviews that met the eligibility criteria. These reviews encompassed a total of 53 meta-analyses and 103 distinct clinical trials. Despite the wealth of research, the researchers found methodological concerns and gaps in knowledge. The quality of the systematic reviews was generally judged to be critically low, with issues such as lack of transparency in study selection and the need for supplementary files listing excluded studies.

Martínez-Calderón highlights the need for future research to focus on improving the quality of systematic reviews and increasing the number of high-quality clinical trials. He suggests using the GRADE approach in all meta-analyses to provide a better understanding of the conditions under which clinical recommendations are made. The GRADE approach is considered the gold standard tool for judging the certainty of outcomes evaluated in systematic reviews, which is essential for implementing clinical research in a clinical setting.

The researchers stress the importance of enhancing the synthesis of evidence in the field of mental health and physical activity, particularly regarding the methodological quality of systematic reviews with meta-analysis. Martínez-Calderón mentions two additional overviews they conducted on exercise for schizophrenia and exercise for posttraumatic stress disorders, where they encountered similar methodological issues.

In conclusion, while exercise-based interventions show promise for addressing psychological symptoms and substance abuse outcomes in individuals with substance abuse or dependence, the existing scientific reviews have methodological flaws and lack clarity regarding the most effective types of exercise for specific populations. Improving the quality of systematic reviews, conducting more high-quality clinical trials, and applying rigorous evaluation tools such as the GRADE approach will contribute to the development of evidence-based clinical recommendations in this field.
