The illegal trade of organs on the black market has become a thriving business, driven by individuals like Joe Boy who are desperate to escape poverty. Among the brokers facilitating this trade is Diane. Meanwhile, those who are forced to rely on dialysis while waiting for a kidney transplant face numerous challenges in their daily lives. The hope for a suitable living donor or a transplant match keeps them going. In Kosovo, investigations into organ trafficking have revealed the country's deep-rooted corruption and organized crime. Due to problems, planned organ transplants in Turkey have had to be relocated to Kosovo. Dr. Yousef, a notorious organ trafficker, is currently on the run. Despite his criminal activities, he is an accomplished transplant surgeon, having successfully performed over 2,000 operations.

His involvement in illegal organ trafficking has earned him nicknames such as Dr. Vulture and the Turkish Frankenstein. Joe Boy finds himself grappling with a difficult decision regarding organ donation. This decision is emotionally complex and requires careful rationalization. Additionally, the familial aspect of needing to help his parents adds another layer of complexity. While considering options for a kidney transplant, Joe Boy feels uneasy about the idea of going overseas and paying for it. He is also deeply affected by the impact of kidney disease on his family members' lives. The ethical concerns surrounding the donation of kidneys for financial gain are significant. Many individuals resort to donating their kidneys out of sheer desperation for money. This has led to the emergence of a black-market organ trade, which poses numerous challenges to the legal system. Mosha Harel played a pivotal role in facilitating this trafficking conspiracy by overseeing blood tests, logistics, and finances. He ensured that medical tests were completed, and donors were made available within a week for the operations. Dr. Zaki Shapiro, a renowned nephrologist, collaborated closely with Harel, making him a crucial link in these life-saving operations.

The Philippines has witnessed a rampant kidney trade, with many individuals selling their kidneys and forming support groups. Unfortunately, these victims find themselves trapped in unregulated and exploitative systems. The consequences of illegal organ trade are dire, as exemplified by Hector's deteriorating kidney health after selling it on the black market, and Mary Joe's eight-year struggle with dialysis. Dr. Yousef SNES has faced accusations of illegal transplantation and organ trafficking, but he has been acquitted in all cases and maintains a clean criminal record. However, he remains at risk of arrest if he leaves Turkey due to an international arrest warrant. Dr. Sonz, on the other hand, justifies his actions as a surgeon based on financial reasons. He appears less concerned about his fate and more preoccupied with the media's portrayal of him, believing that he is merely providing a service to desperate individuals. Doctors are driven by more than just financial gain when it comes to organ trade. Factors such as status and influence also play a significant role in their motives. The compensation offered to kidney donors can vary greatly depending on the economic status of the country.

One way to encourage organ donation is through providing compensation. In a video script, a scenario is depicted where a boy is in danger on a building roof to emphasize the urgency of the situation. The ethical dilemma of compensating kidney donors and the potential for exploitation is addressed. Raul's donor is a crucial figure in Medicu's investigation, as evidenced by a photocopy of Anna's passport found among the seized documents. Anna, a victim of organ trafficking, hails from Moldova. Living on dialysis poses numerous challenges, especially when faced with the uncertainties of the organ transplant list. The availability of organs for donation varies by region, directly impacting the length of the transplant list. Many individuals on dialysis feel fatigued and long to be free from the machine.

The exposure of the organ transplant network has legal implications, leading to Dr. Sones and others facing indictment and police involvement. Under pressure from authorities, ethical dilemmas surrounding organ transplantation come into question. The feeling of being stuck in a time loop is a central theme, highlighting the sense of being trapped in a repetitive cycle. The recurring mention of 'Out of Time' underscores the lack of control over time.
