Death entry is the treatment of the dead human body under IFHRMS. It involves ensuring that all documents are processed correctly and death is recorded in IFHRMS. There are many steps involved in the Death Entry process, which we will be taking you through in this document.

The causes of Death may vary from person to person, but some of the common causes are; Heart Attack, Stroke, Malaria, Dengue etc... The cause often decides the department where you have to take the deceased for treatment. The cause decides which form has to be filled out and what kind of procedure has to follow after death.

The most important thing for you to do is identify the cause of death. Once the cause of death has been established, proceed accordingly.

For example: If the cause of death is a Heart Attack, then check if the deceased has any medical insurance. The family should be informed about this and ask for consent before taking further steps so that they can claim back their policy amount after receiving the certificate from IFHRMS.

If there is no insurance policy, then you can go ahead with treatment at your own will. But if there is an Insurance Policy with IFHRMS, then coordination with DSO (Death and Sales officer) comes into play.

How to do Death Entry in IFHRMS?

Now that you are familiar with the causes of death, let's proceed with the procedures.

Before starting the Death Entry process, check if there are any medical documents that can be used to support your claim. These documents are important for establishing insurance claims so do not ignore them. The medical documents could be letters from hospitals, previous bills etc.

The "Medical Documents" have to be submitted even if there is an insurance policy or not. If there is no insurance policy, then surely you do not need any steps to take prior to Death Entry. But if there is an Insurance Policy, then your procedure should follow this sequence.

In case of a death, you will have to request an estimate from the hospital. The estimation should be done by a competent medical person from the hospital as per your requirement. This estimate has to be submitted along with evidence of the final bill for treatment. In case there is no insurance policy, then you will have to request a final bill separately.

The required documents have been discussed above but still if you face any difficulties, feel free to contact us here for any assistance.

Remember, the Death Entry process is very sensitive. So if you are unsure about any step, do not hesitate to contact us here. We will be happy to help you.

The following are the steps involved in IFHRMS "Death Entry":

Once you have submitted all the required documents, call up DSO (Death and Sales officer) at once. After that, you will have to provide them with an estimate from the hospital along with evidence of the final bill for treatment.

The final bill should be signed by the medical person who advised estimation for your death. The estimate should be signed by anybody who was authorized by you to sign it.

The authorized person can be your relative or an independent medical professional if there is no one available for the authentication of bills.

Any document containing Death Entry information has to be attested by a magistrate or an official seal on it as well as on all other documents submitted with it.

After that, you will be sent the "Death Entry Certificate" after obtaining the required signatures. The DSO (Death and Sales officer) should not send you any other document for this purpose.

Once you receive your Death Entry Certificate, keep it in safe custody. Never allow anybody to take it away from you.

After the Death Entry process is done, take your personal effects and things that belong to your late relative. There are some necessaries which cannot be left with the dead person but you can leave other things like clothes or jewellery etc.

At last, remember that the entire procedure leading up to death is sensitive and do not share this information with anybody unless necessary.

How to overcome the failures in the IFHRMS "Death Entry" process?

If you encounter any kind of difficulty while doing Death Entry, then do not hesitate to contact us here. We will be happy to assist you.

Some of the common reasons for failure in Death Entry are:

If there is no insurance policy at all, then it would be best if you take your relative to a public hospital for treatment.

If the cause of death is a Heart Attack or a Stroke then it's fine to take your relative to a Private hospital for treatment. You can simply treat your relative at your own will and do not have to worry about anything else.

So, in the case of these two scenarios, you do not have to contact the DSO (Death and Sales officer) for anything else.

If the cause of death is some infectious disease like Malaria or Dengue, then you need to take your relative to a public hospital for treatment.

And since there is no insurance policy involved here, you can simply take your relative to that hospital for treatment.

After the Death Entry process is done in IFHRMS, you will get a Death certificate from IFHRMS. You will have to keep this certificate safe in your custody.

You can submit this certificate at any time asked by anybody or the organization.

Conclusion

Death is a sensitive issue and the entire procedure is to be followed in a very precise manner. The coordination with DSO (Death and Sales officers) plays a crucial role in this process. After the entire procedure is done, you will have to keep the Death certificate in safe custody and take whatever your relative or yourself used before death.
