Two complete monsters ready to be dropped into any Dungeons and Dragons 5e campaign.

Fiendish Torturer

When evil souls are sent to one of the Nine Hells to face punishment, it is often the insectoid Fiendish Torturers that are called upon to perform the task – and they enjoy that duty immensely. They have large eyes, a long head, and their insect-like carapace is a dark bluish purple. They are very adept in the use of scalpels and other torture implements, despite their long arms ending in claws.

More Obedience, Less Ambition. Generally, Fiendish Torturers can be found in the service of other, more powerful devils. Even the more powerful among them are content punishing the souls of evil mortals trapped in the Nine Hells, seeking only enough power to be trusted with more valuable souls to torment.

Devil's Sight. Magical darkness doesn't impede the Fiendish Torturer's darkvision.

Magic Resistance. The Fiendish Torturer has advantage on saving throws against spells and other magical effects.

Sadism: For every 10 points of damage the Fiendish Torturer deals in a round, it gains a +1 luck bonus on attack rolls, saving throws, and skill checks during the next round.

Actions

Multiattack. The Fiendish Torturer makes one bite attack and two claw attacks.

Bite. Melee Weapon Attack: +6 to hit, reach 5 ft., one creature. Hit: 6 (1d6 + 3) piercing damage, and the target must succeed on a DC 15 Constitution saving throw or be poisoned for 1 minute. While poisoned in this way, the target can't regain hit points. The target can repeat the saving throw at the end of each of its turns, ending the effect on itself on a success.

Claw. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: 7 (1d8 + 3) slashing damage.

Hunger Demon

Rarely seen outside of the Abyss, the Hunger Demon is the personification of the plane's need to devour souls. The surpringly nimble demon is the size and shape of a morbidly obese orc, with dark gray skin and gaping maws at the end of its taloned hands.

Insaciable Hunger. Woe be to the soul that thinks it has escaped it's demonic tormentors only to find a Hnger Demon wandering the Abyss. The demon wants to do nothing except eat souls, and if they are still attached to living bodies all the better. A Hunger Demon will drop whatever it is doing if it detects a mortal creature. It will follow that creature, and attempt to eat it until either it or its prey has been destroyed.

Magic Resistance. The Hunger Demon has advantage on saving throws against spells and other magical effects.

Sense the Living. The Hunger Demon knows the direction and distance to the closest living creature no matter the distance. If that creature is within 60ft it is not hidden.

Actions

Multiattack. The Hunger Demon makes two claw attacks. If it hits with both it may make a bite attack.

Claw. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 11 (2d6 + 4) slashing damage plus 3 (1d6) necrotic damage.

Bite. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: 7 (1d6 + 4) slashing damage plus 2 (1d4) necrotic damage.

Feeding Tendrils (Recharge 5-6). The Hunger Demon chooses 2 creatures within 10ft. Tendrils of dark energy erupt from its hands and strike the targets. The targets must make must make a DC 15 Strength saving throw. On a failed save, a target takes 7 (2d6) necrotic damage, or half as much on a successful one. The Hunger Demon regains as many hit points as the damage dealt.
