In episode 3, Yoo Si-jin (played by Song Joong-ki) and Kang Mo-yeon (played by Song Hye-kyo) continue to bicker and argue with each other. However, they both begin to see different sides of each other when they rescue a man who had been trapped under the rubble of an earthquake.

Meanwhile, Yoo Si-jin’s team is sent on a mission to a military facility in Urk to retrieve a VIP. While they are there, they discover that the facility has been taken over by terrorists. Yoo Si-jin and his team successfully manage to take down the terrorists and rescue the VIP.

Back in Korea, Kang Mo-yeon is forced to take care of a patient who had been wounded in a gang fight. She realizes that her medical skills are not just a way to advance her career but can make a real difference in people’s lives.

Towards the end of the episode, Yoo Si-jin and Kang Mo-yeon find themselves in a dangerous situation when they are caught in a crossfire between rival gangs. The episode ends with a cliffhanger, leaving viewers wondering if they will survive.

As the opening credits roll, we see a young girl holding a box of tissues, clearly in tears. She walks through a crowded street and enters a building. Cut to a young woman sitting in her bed, staring at her phone. She receives a message from an unknown number that reads, "Can I borrow a tissue?"

The young woman is Kang Mirae (played by Lee Kyu-hyung), a dentist who is about to start a new job in a prestigious dental clinic. She is nervous but excited about this opportunity. As she gets ready for work, she listens to a podcast about preserving oral health. She seems to be knowledgeable and passionate about her profession.

On her way to work, Mirae receives another message from the unknown number that reads, "I know you're a dentist. Can you tell me why my gums bleed every time I brush?" Mirae is confused but intrigued. She responds, "Who are you?"

As Mirae arrives at the clinic, she meets her new boss, Dr. Chun (played by Ko Won-hee). Dr. Chun is a charismatic and confident dentist who seems to have everything under control. She introduces Mirae to her colleagues, including dental hygienist Park Min-kyu (played by Jung Sun-ho) and dental assistant Na Chae-rin (played by Shin Dong-mi).

Mirae's first patient is a nervous young woman named Jang Hae-jin (played by Choi Yu-hwa). Hae-jin has a "big mouth," meaning she has a wide smile that shows a lot of her teeth and gums. She is self-conscious about this and wants Mirae to make her smile less "gummy." Mirae sympathizes with Hae-jin and promises to do her best.

Meanwhile, Mirae continues to receive messages from the unknown number, who identifies himself as Kim Sang-beom (played by Go Min-si). Sang-beom tells Mirae that he has been diagnosed with an oral disease and is desperate for help. Mirae is hesitant to give medical advice over the phone, but Sang-beom seems sincere and persistent.

During her lunch break, Mirae goes to a nearby park and meets Sang-beom for the first time. He is a charming but mysterious guy who seems to have a lot of knowledge about dentistry. He explains his situation to Mirae and shows her his mouth, which has a large sore on the inner cheek. Mirae is shocked and concerned, and decides to bring Sang-beom to the clinic for further examination.

At the clinic, Dr. Chun examines Sang-beom's mouth and confirms that he has a rare and serious infection called noma. She warns Mirae that treating noma is extremely challenging and may require surgery. Sang-beom is devastated and begs Mirae to help him. Mirae feels responsible for getting Sang-beom in this situation and decides to do everything she can to save him.

Mirae spends the rest of the day researching noma and consulting with her colleagues. She learns that noma is a bacterial infection that primarily affects children in developing countries. It is caused by poor oral hygiene, malnourishment, and weak immune system. There is no cure for noma, but early detection and aggressive treatment can prevent it from spreading.

Mirae decides to take on Sang-beom's case despite the risks. She works tirelessly to disinfect and irrigate Sang-beom's mouth, and prescribes him antibiotics and painkillers. She also contacts a noma specialist in the United States for further advice. Sang-beom is amazed by Mirae's dedication and thanks her profoundly.

As the day comes to an end, Mirae reflects on her experience so far. She realizes that dentistry is not just about fixing teeth, but about improving people's lives. She also realizes that she has a lot to learn, and that she can't do it alone. She texts the unknown number and says, "Can we meet again? I need your help."

The episode ends with a montage of Mirae treating various patients, including Hae-jin, who is thrilled with her new smile. Mirae narrates, "The mouth is a doorway to the body, but also to the soul. Every mouth has a story, and every story deserves to be heard."

Overall, this first episode of Big Mouth is engaging, informative, and emotional. The characters are likable and relatable, and the plot is both educational and entertaining. The show manages to tackle serious oral health issues with sensitivity and humor, and leaves the audience wanting more.
