Best Underwear for Men: Can Boxers or Briefs Affect Sperm Health and Fertility?
What science says about tight underwear, testicular heat, gym wear, sleeping without underwear, and male performance

Men’s underwear is usually purchased based on comfort, appearance, price, or brand. However, many men also wonder whether wearing tight underwear can affect sperm health, fertility, testosterone, sexual stamina, or performance in bed.
You may have heard claims that briefs “damage sperm,” boxers increase fertility, or tight underwear reduces the size of the male reproductive organs. The reality is far less dramatic.
Underwear may influence temperature, sweating, irritation, and everyday comfort. However, it does not directly control a man’s sexual stamina, penis size, masculinity, or ability to perform in bed.
Here is what men should actually know before choosing their next pair.
Why Are the Testicles Outside the Body?
The testicles are positioned outside the main abdominal cavity because sperm production works best at a temperature slightly lower than the body’s core temperature.
When the scrotal area experiences excessive or prolonged heat, sperm production and sperm quality may temporarily decline. Research on testicular heat stress has linked excessive heat exposure with reductions in sperm production, movement, and normal development.
This is why men who are trying to conceive are sometimes advised to reduce unnecessary heat exposure from hot tubs, saunas, heated seats, laptops placed directly on the lap, and extremely tight clothing.
However, underwear is only one small part of the fertility picture.
Smoking, heavy alcohol consumption, obesity, certain medications, anabolic steroids, infections, medical conditions, chemical exposure, age, stress, and overall health can also influence sperm quality.
Boxers vs Briefs: Which Is Better for Sperm Health?
Loose-fitting boxers allow more space around the scrotum and may provide better ventilation than close-fitting briefs.
A study involving 656 men attending a fertility centre found that men who mainly wore boxers had higher average sperm concentration and total sperm count than men who usually wore tighter underwear. However, the study was observational, meaning it found an association but could not prove that underwear alone caused the difference.
A more recent scientific review reached a cautious conclusion. Tight-fitting underwear may raise scrotal temperature, and some studies have found modest differences in semen measurements. However, the evidence remains inconsistent, and studies have not clearly demonstrated that underwear choice changes a couple’s chance of achieving pregnancy.
Therefore, wearing briefs does not automatically make a man infertile. Switching to loose boxers may be a reasonable and inexpensive step for men who are actively trying to conceive, especially when combined with other healthy lifestyle habits.
Understanding the Main Types of Men’s Underwear
Briefs
Briefs offer firm support and leave the thighs uncovered. They work well under formal trousers, slim-fit clothing, and during activities where additional support is helpful.
The problem is not the word “brief.” The concern is underwear that is excessively tight, traps heat, leaves deep marks, causes pain, or constantly presses the testicles against the body.
A properly fitted brief that feels comfortable is unlikely to destroy a healthy man’s fertility.
Boxer Briefs and Trunks
Boxer briefs combine the support of briefs with the thigh coverage of boxers. Trunks are generally similar but have shorter legs.
These are practical everyday choices for men who experience thigh rubbing or prefer more support than loose boxers provide. Choose a fit that holds everything comfortably without excessive compression.
Loose Boxers
Traditional boxers provide greater airflow and less compression. They may be the most practical option for men who are concerned about heat exposure or are trying to improve every controllable aspect of their reproductive health.
However, they may not provide enough support for running, gym training, cycling, or other high-impact activities.
Compression Shorts and Jockstraps
Compression underwear and jockstraps can provide valuable support during exercise. They are designed for physical activity, not necessarily for wearing continuously throughout the entire day and night.
After a sweaty workout, shower and change into clean, dry underwear rather than remaining in damp gym clothing. Fungi responsible for conditions such as jock itch thrive in warm, moist environments covered by tight clothing.
Is Cotton Always Better Than Nylon or Polyester?
Cotton is soft, breathable, and usually comfortable for everyday wear. It can be particularly helpful for men with sensitive or easily irritated skin.
Performance fabrics containing nylon, polyester, or elastane may dry faster and reduce friction during workouts. However, no strong human evidence proves that one particular underwear fabric significantly improves fertility under normal conditions. Current reviews describe evidence about fabric composition as limited.
Instead of obsessing over one material, consider three practical factors:
Does the underwear fit comfortably?
Does it keep the area reasonably dry?
Can you change it after heavy sweating?
Cleanliness, dryness, and fit are generally more important than marketing terms printed on the packaging.
Should Men Sleep Without Underwear?
There is no medical rule requiring every man to sleep naked.
Sleeping without underwear, wearing loose boxers, or wearing loose pyjama bottoms may feel cooler and more comfortable. It can also reduce friction for men who experience sweating or irritation.
However, sleeping without underwear has not been proven to increase testosterone, boost sexual stamina, enlarge the testicles, or guarantee better sperm quality.
Choose whichever option allows you to sleep comfortably while keeping your bedding and skin clean.
Can Tight Underwear Reduce Sexual Stamina?
Underwear does not directly determine sexual stamina or erectile performance.
An uncomfortable or extremely tight garment may cause heat, pressure, chafing, numbness, or distraction. Removing that discomfort could make someone feel more confident, but it is not the same as medically increasing stamina.
Persistent difficulty achieving or maintaining an erection, reduced sexual desire, pain, infertility, or major changes in ejaculation should be discussed with a qualified healthcare professional rather than treated by changing underwear alone.
What About Odour, Dark Skin, and Intimate Whitening Products?
Some natural darkening in the groin is common because of genetics, hormones, friction, sweating, and skin type. It does not mean that the area is dirty.
Products promising to “whiten” intimate skin or eliminate every odour within a fixed number of days should be treated cautiously. Fragranced deodorants, harsh acids, bleaching ingredients, and unverified serums can irritate sensitive genital skin. The American Academy of Dermatology advises against applying fragranced products directly around the groin.
Wash gently, dry the area properly, change underwear daily, and change quickly after sweating. Seek medical advice for a persistent strong smell, discharge, painful rash, sores, swelling, severe itching, or recurring infection.
The Final Verdict
For everyday office work, choose breathable briefs, trunks, boxer briefs, or loose boxers that do not feel excessively tight.
For workouts, use supportive and quick-drying underwear, but change out of it after exercising.
For sleep, loose clothing or no underwear is acceptable according to personal comfort.
For men trying to conceive, loose-fitting boxers and avoiding excessive heat may be sensible precautions. However, underwear alone cannot cure infertility or guarantee healthy sperm.
The best underwear for men is not a magical fertility product. It is simply underwear that fits properly, prevents unnecessary overheating, keeps the skin comfortable, and suits the activity being performed.
