Researchers found the removal of one of the two adrenal glands helped treat hypertension
A DNA mutation that results in high blood pressure has been identified by researchers, along with a remedy for it. “Nature Genetics,” a journal, published these findings. One in twenty people with hypertension has a small benign nodule as the root cause. Aldosterone, a hormone that controls the body’s salt levels, is produced by the nodule. The most recent discovery is that some of these nodules have a gene mutation that results in enormous, sporadic overproduction of the hormone.
The discovered gene mutation results in a number of problems, which makes it challenging for clinicians to diagnose some hypertension patients. A protein called CADM1 is first affected by the mutation because it stops cells from communicating with one another and alerts the body when it is time to stop making aldosterone. The fluctuating aldosterone release throughout the day, which causes salt excess and hypertension at its worst, is of special concern to medical professionals. It is because of this variety that individuals with the gene mutation can go unnoticed until they undergo blood testing at various times throughout the day.

The researchers also learned that a unilateral adrenalectomy, or the removal of just one of the two adrenal glands, could treat this particular type of hypertension.
After removal, severe hypertension that had been resistant to several pharmacological treatments vanished, and numerous years of observation later, no further treatment was necessary.
Because aldosterone is not regularly assessed as a potential cause, less than 1% of patients with hypertension brought on by aldosterone are recognised as having it. Instead of one-time blood samples, the researchers advise measuring aldosterone throughout the course of a 24-hour period in order to identify more people with undetected hypertension.
The first participant in this study was identified when medical professionals saw a change in his hormone levels when he was taking part in a clinical trial of medications for severe hypertension.
The source of hypertension in the majority of patients, who need lifelong medication treatment, is unknown. A gene mutation in the adrenal glands causes excessive levels of aldosterone to be produced, which is the cause of hypertension in 5–10% of cases, according to earlier studies by the Queen Mary group. Aldosterone makes the body retain salt, which raises blood pressure. Patients who have elevated blood levels of aldosterone are more susceptible to heart attacks and strokes and are resistant to treatment with routinely prescribed medications for high blood pressure.

On the occasion of the 900th anniversary of Barts Hospital, Professor Morris Brown, co-senior author of the study and Professor of Endocrine Hypertension at Queen Mary University of London, said, “This narrative highlights advantages from the virtuous circle between science and medicine. The majority of patients give their consent for us to perform non-routine molecular analyses on surgical samples, which allow us to understand how their hypertension developed and how to treat it in future patients. We are currently examining if brief cauterization of the aldosterone nodule provides an alternative to surgically removing the entire adrenal gland because the aldosterone nodules in this trial were so small.
