The coccygeal nerve, also known as the coccygeus nerve or the fourth coccygeal nerve, is a spinal nerve originating from the coccygeal plexus in the lower spine. The Coccygeal Nerve is a part of the sacral plexus and consists of several branches. This nerve provides sensory innervation to the skin and muscles around the coccyx (tailbone) and the perianal region. Its functions include transmitting pain and touch sensations from these areas to the brain.

Disorders affecting the nerve, which is part of the sacral plexus, encompass a range of conditions. Although these disorders are relatively uncommon, they can result in pain and discomfort. Various types of nerve disorders include;

Different Types of Coccygeal Nerve Disorders

Coccydynia. This coccygeal nerve disorder is characterised by pain and tenderness in the tailbone or coccyx region. It can be caused by trauma, injury or inflammation affecting the coccyx.

Pudendal Nerve Entrapment. The pudendal nerve runs close to the coccyx. When it becomes trapped or compressed, it can lead to pain, genital discomfort and distress in the coccyx area.

Coccygeal Neuropathy. This rare condition results from damage or dysfunction of the nerve, leading to pain in the tailbone region.

Sacral Tarlov Cysts. While these cysts typically affect nerve roots, there are instances where they may involve the nerve as well; this involvement can cause pain and discomfort in the region of the tailbone.

Posterior Pelvic Pain. Certain conditions, like pregnancy-related issues or muscle imbalances can contribute to pain that might radiate towards the region of the coccyx.

There are ways to treat disorders of the nerve. These options include methods such as therapy and pain management or, in more severe cases, surgical procedures that aim to relieve symptoms and enhance the patients’ overall well-being.

Causes of Coccygeal Nerve Disorders

Acute trauma is by far the most common cause of tailbone pain. Infections, osteoarthritis, and bone spurs are further potential causes of coccydynia. The coccyx bones, the discs between the spinal segments, or the nearby muscles and ligaments may be the source of the discomfort.

Acute Trauma – A sudden injury to your tailbone may be the source of this type of pain.

Repetitive Stress Injury – Tailbone discomfort can develop over time from activities such as riding a horse, cycling, or simply spending a lot of time in a bumpy car. It might be attributed to the repetitive strain or rubbing of the tailbone caused by these activities.

Childbirth – Pain that lasts longer after vaginal delivery might result from pressure on the tailbone. It is particularly valid in cases when the delivery is challenging. It is because the baby’s head compresses the top of the tailbone.

Bone Spurs – An isolated bony growth is called a bone spur. Bony spurs near the coccyx can compress or irritate the coccygeal nerve, leading to pain, discomfort, and coccygeal nerve-related symptoms.

Nerve Pain – Inflamed or compressed nerves may also cause tailbone pain. One possible cause of nerve discomfort at the top of the tailbone is the ganglion impar, a bundle of nerves.

Hypermobility – Tailbone discomfort is a possible symptom of joint hypermobility syndrome. A condition known as double-jointedness, or joint hypermobility, is brought on by loose connective tissue, which permits joints to move beyond their natural range of motion.

Symptoms of Coccygeal Nerve Issues

A few possible signs of coccydynia are:

Tailbone ache that is either dull (achy) or acute (piercing).

Backache that becomes worse when you rise after sitting down.

Suffering from diarrhoea.

Experiencing pain during intercourse.

Treatment Options For Coccygeal Nerve Issues

Lifestyle Modifications- Most causes of tailbone pain can be relieved with lifestyle modifications, allowing the condition to repair itself. Healing might require some time. The healing time of a bruised tailbone might range from a few days to several weeks.

Medications- Nonsteroidal anti-inflammatory drugs (NSAIDs) may be prescribed orally or topically by your doctor. This may lessen inflammation and discomfort. Your doctor could recommend a stronger painkiller if the at-home treatment doesn’t relieve discomfort.

Physical Therapy- Physical treatment can assist if pelvic floor muscle spasms are the source of your pain. It frequently consists of:

Pilates for your pelvic floor, or reverse Kegels
