The basic and the most important are - demographic proof, social security number, educational certificates, proof of license, proof of insurance, etc.

Select an insurer - Different insurers provide different cover. Deciding on which insurance company to choose depends on the services offered by the practitioner.

CAQH - It is essential to apply for the Council for Affordable Quality Healthcare. Having a CAQH number is essential before applying for the credential process.

Following up / Certification - Following up is essential to ensure the process completes within a stipulated time. In case of a mistake in the process, there may be a delay of at least a week or even more.
