Jessica, Malorie's sister, pays her a visit five years prior while she is pregnant. Unaccounted-for mass suicides in Europe and Asia are being reported via a news broadcast. Malorie watches as a woman repeatedly smashes her head against the hospital window following a pregnancy appointment. There is panic and mayhem when other people also exhibit suicide behaviour. Malorie then quickly leaves the area with Jessica. As Jessica drives away from the hospital, she notices something strange that makes her crazy and causes her car to crash on purpose. She is then slain when she gets killed by a speeding truck.

A woman offers to let Malorie stay in her home while she runs away on foot. After that, the woman sees the creature, enters a trance, and sits inside a burning car. Malorie is picked up by Tom, a bystander, off the street and brought inside the home where she is joined by Douglas, Greg, Cheryl, Felix, Charlie, and Lucy. Charlie claims that the appearance of demonic entities is a portent of the end times and that humanity has been judged. He also says that those spirits go by different names in different cultures, as Púca, Surgat, Huli Jing, and Aka Manah. Every time someone leaves the house, they blindfold themselves and cover all the windows. Greg kills himself when Olympia, a new survivor who is pregnant, comes. Greg volunteered to see if it is safe to watch them through security camera surveillance.

To replenish their running low on food, half of the group heads to the grocery shop where Charlie used to work. They use a GPS navigation device to navigate there while driving a car with covered windows. Malorie receives their supplies and three pet birds. She sees that the birds are disturbed by the creatures' presence. Afterwards, an attempt is made to get the group look at the animals by Charlie's coworker, who attacks them. Charlie, though, gives his life to save the others. They return safely to the house. Felix and Lucy eventually steal the vehicle and take off.

Against Douglas's desires, Olympia welcomes a stranger named Gary into the house. Gary explains that he escaped from a group of survivors who had not been blindfolded and were trying to make others look at the creatures since they were nuts from viewing them. Douglas tries to make Gary leave, but Gary is locked in the garage by the others after Douglas is knocked unconscious by Cheryl. While Malorie and Olympia go into labour at the same time, Gary draws the entities, indicating he has also seen them and has gone mad. After rendering Tom comatose, he lets Douglas see the creatures by opening the garage door. Gary goes upstairs and tears the blinds on every window. Olympia doesn't dare turn away, and upon seeing the thing, she jumps out of the window and ends her own life. While Gary coerces Cheryl to look at the entities and ultimately causes her to stab herself in the throat with a pair of scissors, Malorie hides with both of the newborn infants under a cover. Douglas breaks out of the garage, tries to shoot Gary with a shotgun, inadvertently hurting him, but Gary kills him with the scissors. Soon after, Tom starts to heal while battling Gary for the pistol. Malorie hears many gunshots, but Tom reassures her that nothing is wrong.

Five years later, Tom and Malorie share a home with "Boy" and "Girl," their children. Survivors send messages to them about a safe settlement concealed in the forest that can only be reached by boat down the river. A group of survivors who have not been blindfolded attacks them as they leave their home. Malorie and the kids are able to escape as Tom diverts the attackers. When he opens his eyes, he sees the entities, but before he kills himself, he takes out every attacker.

Using the birds as a warning against the entities, Malorie and the kids ride down the river in a boat while wearing blindfolds. A blindfolded survivor and swift-moving river are among the many challenges they face. Malorie unexpectedly slips down a slope, separating the three of them shortly after they arrive at the coast. Nearly removing their blindfolds, the entities deceive the children. When she comes to, Malorie tells the kids where to find her and that she loves them. At last, they arrive at the village, which used to be a school for the blind. After giving the birds their freedom, Malorie reveals her true identity as their mother by naming the kids Tom and Olympia. Upon realising that Dr. Lapham, her obstetrician, is among the survivors, she is even more delighted.
