In 2018, a copy of Antrum resurfaces, and a documentary crew decides to make a short film on its history and impact. The documentary crew finds out that unrelated, black-and-white snippets of an apparent snuff film have been spliced into the original movie by a third party. The crew pauses the documentary so that Antrum can be presented in its entirety for the first time in twenty-five years.

The movie's central characters are siblings Oralee and Nathan, who have recently lost their pet dog, Maxine. Nathan starts experiencing disturbing dreams and visions of demons after the pet dog is euthanized. Oralee uses a grimoire to guide Nathan through a series of rituals, intending to dig a hole to Hell to rescue Maxine. The siblings enter a nearby forest, where Satan is believed to have fallen to Earth, and are met with unforeseen consequences.

The filmmakers use a variety of techniques, including disorienting sounds and subliminal imagery, to create a creepy and unsettling atmosphere.

One of the most interesting aspects of Antrum is the way in which it blurs the lines between reality and fiction. The film is presented as a documentary, complete with interviews with supposed experts and footage of the filmmakers conducting research. However, it quickly becomes apparent that much of what we are seeing is not real, and that the filmmakers are playing with our expectations and assumptions.
